Team Formation Meeting Info

Players who accept an offer to participate on a Classics FC team should plan on attending a Team Formation meeting the week of June 11, 2007 at Liberty Middle School. All details are listed below.

During these meetings we will introduce the the club staff and coaches, discuss the team calendar for 2007-08 including the league games, tournaments, practices and camps, and review the uniform ordering process.

When you come to this meeting, please bring the following:
1. Classics FC Commitment Fee (U9-U14): $350
Delaware County Commitment Fee (U15-U18): $200

Checks (personal, cashier, money order) should be made payable to OYAA. No cash, debit or credit cards please.


2. Photographs: two (2) 1" x 1" photos of your child. Photographs are glued to a league player card that is only 2" x 3".


3. Birth Certificate: A government issued copy of your child's birth certificate to verify age. Exception: if your child played on a Classics FC team in '06-07 we can use their player card.


4. Medical Release Form: A completed Medical Release Form. The form can be found on the Classics FC website at: MedicalReleaseForm.pdf

This form must be notarized. The club has arranged for notary public to attend each team formation meeting.

Directions: Liberty Middle School is located on Liberty Road approximately one (1) mile south of Home Road; or three (3) miles north of Powell Road. The school sits next to the Powell YMCA.
